SKYEHEALTH MOBILITY Rental Agreement Terms

This Rental Agreement (“Agreement”) is made between SkyeHealth Mobility (‘Company”) and the Customer (“Customer”) for the rental of Mobility Scooters, Mobility Wheelchairs, Standard Wheelchairs, Rollators, Electric Hospital Beds, Knee Scooters, and other Durable Medical Equipment (“DME”), By renting equipment from SkyeHealth Mobility, the Customer agrees to the following terms and conditions:

  1. Rental Period and Payment

Rental Duration: Equipment may be rented on a Per Day, Weekly, or Monthly basis.

Payment: Full payment must be made via credit card or cash prior to receiving the equipment.

Late Returns: If the equipment is returned more than 2 hours late, a $25 late fee will be charged to the Customer’s credit card.

Delivery and Pickup

Delivery and Pickup: SkyeHealth Mobility will deliver the equipment to the Customer’s location and pick it up from the final destination.

Operating Hours: Delivery and pickups are available between 8:00 AM and 8:00 PM.

Online Orders: Orders can be placed online 24 hours a day.

Equipment Use and Liability

Age Requirement: Customers must be 18 years or older to rent any equipment.

Responsibility: SkyeHealth Mobility is not responsible or liable for any injuries that my occur while operating the equipment.

Care of Equipment: Customers must use the equipment as intended and take reasonable care of it.

Damages and Repairs

Inspection: Equipment will be inspected upon return.

Damage Assessment: If any damage is found, the Customer will be notified, and an assessment will be made.

Payment for Damages: The Customer must pay for any damages in full within 7 business days of the notification.

Returns and Cancellations

Returns: Equipment must be returned in the condition it was rented.

Late Fees: A $25 late fee will be applied for returns more than 2 hours late.

Cancellation: Customers may cancel their order 24 hours before delivery for a full refund. If cancellation occurs after delivery, a prorated rental fee and pickup charge may apply.

General Terms

Amendments: SkyeHealth Mobility reserves the right to amend these terms at any time.

Governing Law: This Agreement shall be governed by the laws of the state in which SkyeHealth Mobility operates.

By renting equipment from SkyeHealth Mobility, the Customer acknowledges that they have read, understood, and agree to be bound by terms and conditions set forth in the Agreement.
